If the form is to be used to view/update existing records, then it could
just a logically be D, allowing only existing customers, but could also
still be B for the update.

If the form is to be used for new customer entry, then there wouldn't be
a value for customer number - that should be generated by the system to
ensure uniqueness - therefore, the correct answer for all uses of the
form would be B

I have to disagree with Mark. The ATO online Tax Pack provides a drop
down

list of titles that is consistent with AGSM usage. The Australian

Government Style Manual has about 3-4 pages on titles and their use, in

the Austrailian context, is regarded as exhaustive. So, a drop down list

of titles has good precedence, even though it might be cumbersome. As
for

extracting the drop down list from a lookup list or a dynamic table;
this

is quite a trivial task with Access, SQL/PHP or SQL/Perl and one that I
do

when I put student grades online for the college admin.
