[Busmanagers] Permission Notification Report for Excursions SG21002

If any school generates the Permission Notification Report for
Excursions SG21002 for Groups,

Could you please advise the procedure to retain the signed consent form
at school and to give a copy to the teacher going on the excursion?

The Student Safety & Risk Management (last updated September 2005) under
4.4.2.4 states the above.

 

 

Permission Notification Report for Excursions SG21002 only has one tear
off slip, by the time it comes back signed it will be difficult to
photocopy the slips.
