[vet-mm] Authentication record form

Yes I found this too when students were applying for TOP Designs. They require an Authentication Record Form that does not exist in the VCAA requirements for VET IDM as far as I am aware. However I adapted and used my own Authentication Record this year for each Assessment Task. So I copied these for the submission.
