[vet-mm] Authentication record form

Hi all,

One of my students is wanting to put his IDM stuff in for Top Designs (I know it's due tomorrow but he has only just come to me). The form asks if the Authentication Form is included.

Am I going crazy but as far as I knew we didn't have an actual Authentication Form like the other creative subjects? I've looked all over the place just in case, but can't find anything.

Does anyone know what this is asking for?
