[Year 12 SofDev] Usability forms

Hi all,

For those of you that have never done usability before there is usually a little bit of paper work involved. The really important one is a consent from signed by the victim. So we don't have to reinvent the wheel the lecture for User-Cantered Design at Swinburne University has given me a template consent form that I can pass onto this list. I have changed the template so it is generic or you will need to do is fill out the appropriate details before handing it onto your students to do the finishing touches.

I have also attached some questioner templates for demographics and satisfaction that you can use as well as a template task sheet the students can use. Along with all of these template I have attached pdf files that guide you through how to set up and modify all the templates. Again I would like to thank Swinburne for these resources.

Beware that the students at Swinburne University go through the full usability testing experience including using an actual usability lab so some of the information on the templates may not be applicable for your students.
