That's where finding the difference between the problem and the actual 
issue is vital
in determining client requirements.
eg; the problem is pimples, the real issue is hormonal or dietary
or
the problem is the website, but the real issue is server robustness.

it's also useful to restate the client's requirements in conversation in 
a 'what i hear you say is...' type scenario.
it can avoid a lot of misunderstanding.
