As you say - real life problems, real life people and mistakes!
So why do we design first?
Because of course we have some idea of what we want to do and we "design" the database to do that. As we work on the development of the database we find that we have not thought things out fully, or we have made silly mistakes. So we make changes to our original design "on the fly".
Document the changes. No problem.

Hello,
Three of my students have just realsied they have designed their data tables with some errors in them. They relaised this as they're entring some data.
Is it appropriate to for them to annotate the corrections on their original designs to show they've had to go back and "redesign" some elements?
