[Year 12 IT Apps] Internet Student Usage Form

Good morning everyone!

Does any one have an updated version of a form for students to sign off on
that states what they can and cannot do using the Internet?

I have been using one created by Stan Szydzik from Ballarat Secondary
College and modified for where I teach in TAFE
