Hi Glynis,

If you have a look in edulibrary, you will find the Confidential Medical
Information For School Council Approved Excursions.  The indemnity is on
the bottom of this form.

Hi Everyone

Can anyone tell me where I can find a copy of the indemnity form that
goes on the bottom of excursion notices?  We want to see if the one we
are using is OK.
