You could try SMC21101 (Student Medical Report) or ST21020 (Student
Disaster Plan)

Hi everyone,

Does anyone know if it is possible to print off a report from Cases that
gives medical, immunisation, contact details, medicare number etc?
Apart from the full student enrolment report, that's 4 pages long, there
doesn't seem to be one that will provide this information which would be
useful to take on excursions, or to take with us if we need to take a
child to the doctor or hospital.  We know there's a student medical
report, but that will only print if the child has a recorded medical
condition.
