We mark the groups with permission received in the relevant column and
monies paid, Print report. Give teachers the original permissions for
the day of

Excursion and when they return they give them back to us for keeping.

Our teachers have also all relevant medical reports printed for the
children and given that on the day.

Permission Notification Report for Excursions SG21002

 

If any school generates the Permission Notification Report for
Excursions SG21002 for Groups,

Could you please advise the procedure to retain the signed consent form
at school and to give a copy to the teacher going on the excursion?

The Student Safety & Risk Management (last updated September 2005) under
4.4.2.4 states the above.

 

 

Permission Notification Report for Excursions SG21002 only has one tear
off slip, by the time it comes back signed it will be difficult to
photocopy the slips.

Can anyone tell me if there is an Unsupervised Excursion form on softweb
and if so where to find it.  I have searched and searched to no avail.

We currently use one but I am not sure how current it is as it is very
very old.
