[Busmanagers] Cessation of duties Form

Helen

The cessation advice is triggered by the termination information on
HRMS.  All that is required from the school end as far as notification
to the GSO is to enter the termination on HRMS.  If this has been done
and the GSO says that they have not been informed, I need to know the
details so I can follow it up.

The Cessation of Duties form is only relevant to request payment in lieu
of unused leave entitlements i.e. LSL and/or Annual Leave (SSOs only).

Update

Have checked with HR  the 'Cessation of Duties' form (not Completion of
Service form) automatically is generated at DE&T Melbourne and sent
across to the Super Board.

Still can't find a leave form on HR though!

Chris,

We have a retired staff member who has received a letter from the Super
Board to say they can not release his super until they receive a
'Cessation of Duties' Form. I understand this form is processed from
DE&T Melbourne. Can you confirm that is correct please.
