Couldn't find form on HRweb - but this could be me as I can't find a
basic 'application for leave form' anymore -

Yes that is correct and you can find the form on HRMS under forms

We have a retired staff member who has received a letter from the Super
Board to say they can not release his super until they receive a
'Cessation of Duties' Form. I understand this form is processed from
DE&T Melbourne. Can you confirm that is correct please.
