You must have a tick in the Tax Invoice box to obtain a tax invoice. The
tax invoice box on Sundry Debtors is on the Financial Summary screen in
DR11001 Debtor Information. 
The same issue arises for Parents which also need the tick Requires tax
invoice set to Y on DF11003 Aged Balances. (There is a problem that you
cannot actually put that tick in until after they have had at least one
invoice - so you must make sure that the first invoice does not need a
tax invoice)
