[Busmanagers] Direct Deposit form

Hi

 

I have looked in the A-Z forms section of HRMS and I cannot find the
direct deposit form which gives you the choice of more than one account
to have your salary deposited in to. I haven't got it saved anywhere
either unfortunately.

 

Can someone please email me the form, or tell me where to find it.
