[Busmanagers] Medication for students

We are looking at having a proforma that parents have to fill in
recording medication that their child is on, dosage, time given etc as
we have had a child recently who wasn't given his medication.  Does
anyone have this set up for us to have a look at.  Our AP has done a
draft but thought if there was other things around it might have
something we have missed.

Also we have a form that each teacher fills in for CRT's when they are
away.  Does anyone use this type of form and could show us.
