[Busmanagers] Maternity Leave / Family Leave Forms

Hi

 

Just off the top of my head I thought the person had to write a letter
to the principal and provide a medical certificate with due date of
confinement.  Then a normal leave form is used.  If you look up HR web
it has instructions regarding the documentation required.

Hi everyone 

Can anyone point me in the right direction to find Leave forms for paid
maternity leave and unpaid family leave.  I have looked under "F" on the
HR web site but can't find anything.

Do we need to create our own?  If so, does anyone have one I can copy?
